In recent years, an increasing number of medical technologies such as devices, diagnostics, and digital health technologies have been evaluated through various health technology assessment (HTA) frameworks around the world.

Because assessment by HTA organizations is still evolving, there is a lack of harmonization among various agencies on process and methods. Information may seem unclear - particularly the requirements for economic evaluations - making planning for submissions difficult for companies developing these technologies.

To address this, we developed an online survey requesting information from various HTA agencies on their key health economic evaluation metrics.

Our results show that there are differences and similarities among the various HTA organizations, in aspects such as topics covered in the economic systematic literature reviews, type of economic evaluations considered, perspectives used, discounting rates used for cost and outcomes, and willingness-to-pay thresholds. Economic assessment is linked to national contexts and understanding these key differences and similarities is essential if companies are to demonstrate economic benefits efficiently and achieve market-access objectives for their products.

While health technology assessments, or HTAs, of pharmaceutical products have existed for a while, HTA organizations have in recent years taken medical technologies under their wings as well. Nordic countries submissions are often grouped together for efficiency. However, the assessment of medtech is a developing area, and there is no current consensus on methods.

We conducted a study to assess the range of medtech processes and requirements in Nordic countries. We developed an online survey for Nordic HTA organizations requesting information on the selection process, general submissions process, and types of evidence considered part of the clinical and economic assessment.

We discovered that there are many similarities between the organizations that could provide efficiencies. However, medtech companies must be aware of the differences – especially in the selection process – that will impact evidence generation needs and submission planning.

In this study, we identified challenges in designing conceptual models for biologic therapies in eosinophilic-associated diseases.

We performed a targeted search of the Embase database and health technology assessment agency websites to identify examples of cost-effectiveness modeling. The model structures often included health states defined by levels of disease and treatment response. However, vague definitions of disease control and a lack of consensus on treatment response posed a challenge in evaluating comparative treatment efficacy. Other challenges included understanding long-term treatment efficacy, identifying appropriate treatment stopping rules, finding publications reporting stratified cost and utility estimates, and difficulty quantifying the costs and disutility of long-term steroids use.

We recommend early engagement with clinical experts and payers is recommended for reaching a consensus on positioning biologics and creating clear definitions of disease control and treatment response. Further research on resource use and utility associated with different levels of disease activity is needed.
